    Position Summary:


    The Community Liaison will interact, establish, and maintain relationships with health care professionals as well as identified key referral sources to attract new business referrals for the community.  In addition, the Community Liaison will help follow residents throughout their transition of care from home to hospital to skilled nursing facility/rehab and back home.  The Community Liaison will provide resources/communication to Resident/Family/Community Management team to help ensure a safe return back to the community.


    Required Experience:

    • Currently Licensed LPN, Clinician or MSW
    • Bachelor's degree and related healthcare experience and knowledge preferred
    • Minimum of two (2) to five (5) years of recent home health sales experience preferred
    • Excellent communication, negotiation and public relations skills
    • Ability to market effectively with customers, referral sources, and the community
    • Compliance with accepted professional standards and practices
    • Self-directed with the ability to work with little supervision
    • Flexible and cooperative in fulfilling all obligations
    • Present professional appearance/dress
    • Maintain a positive attitude which supports team performance and productivity
    • Must exhibit a high degree of initiative and creativity
    • Must be highly organized, able to prioritize, and multi-task in a fast-paced environment
    • Previous experience with a CRM tool such as Salesforce preferred

    Primary Responsibilities:

    • Community Liaison identifies opportunities for relationships with physicians and organizations
    • Conduct outreach and develop ongoing relationships with referral agencies, community/professional organizations and other sources
    • Convert universe to traffic and drives traffic to property via local events, campaigns, advertising, and networking efforts
    • Seeks new opportunities to expand our growing number of referral sources
    • Follows up on potential referrals and cross sells other internal services with Leasing Consultant.
    • Attend relevant networking and marketing events in order to expand business contacts and relationships
    • Maintains awareness of open inventory, overall market pricing and occupancy and revenue goals for the community
    • Works with staff to ensure that new business successfully goes to start of care
    • Community Liaison provides ongoing contact with family and/or responsible party to obtain consent to assess/follow patient; coordinates between physician and patient about changes in plan of care; acts as patient advocate with external medical care providers
    • Establishes home care plans before discharge from a hospital or rehab facility
    • Adheres to modified coaching model to include bedside teaching to patient and family across the care continuum specifically medication reconciliation, personal health record, red flags for risk of readmission and how to manage, and securing follow up appointment with primary care physician to occur within 5 to 7 days of patient discharge to home
    • Coordinates patient services and equipment needs
    • Proactively identifies and assesses needs of client and community
    • Carrying out supervisory responsibilities in accordance with the organizations policies and applicable laws
    • Completion of appropriate paperwork and recommendations as to better and/or more efficient ways of operating the department
    • Community Liaison holds knowledge of all emergency policies and provision of updated information to the Wellness Staff
    • Performs other similar or related duties as necessary
    • Work toward continual improvement of the overall organization
